There are 3 types of booking forms, each requiring different information. If you wish to make a booking for manciures, pedicures, facials, waxing, general make-up & hair-styling, spray tanning, personal styling, education, massages etc, complete a General Beauty Booking Form . If you wish to book for a wedding, complete the Wedding Booking Form or a Spartie, complete the Spartie Booking Form.
